We’re an unregistered NDIS provider, so we can’t take NDIA-managed (agency-managed) plans.

Documents to have ready
Upload whatever you have – not everyone will have all of these: NDIS plan, functional capacity assessment, therapy reports (dietitian, physio, OT, speech), PBS plan, and any other letters or plans.
